PLACING AN ORDER or TRANSFERRING SERVICE

Orders may be submitted on any
document, such as a purchase order, price list,
letterhead, renewal form, or even a renewal form
from other agencies.
When using alternatives to the WT Cox
Subscriptions order form, please include:
- A list of the new periodicals you wish to order,
- The quantity of each title,
- A list of renewal titles with current expiration dates,
- A start date for each subscription,
- A name or department for each title,
- An Authorizing signature, and
- A ship-to and bill-to address.
